Community Outreach
Community health workers (CHWs) or outreach workers are responsible for brief case management (typically less than 90 days) for HIV positive persons who are recently diagnosed, lost to care or tenuously linked to care.
CHWs also identify individuals who know they are HIV positive, but are not in medical care by receiving referrals and using information about missed appointments. The CHWs also work with Inova Juniper Program inter-disciplinary teams to assist clients with referrals to mental health and substance abuse treatment, housing programs and education and employment services.
