Phone – New Patients
Call our referral coordinator, Andrea Grajeda, at 703-776-6168 during business hours to set up your initial appointment. You may also send an email to andrea.grajeda@inova.org.
When you schedule an appointment, in addition to your name, address and phone numbers, you will be asked to provide the following information:
- The names and phone numbers of your referring physician, your primary care physician and any specialists you are seeing.
- The names and phones numbers of your next of kin or emergency contact.
- Your insurance information, including the 800 number for provider services.
- Please identify any special needs that you or a family member has who is coming with you. Services are available for patients with hearing, vision or mobility needs. Upon request, interpreters are available for non-English speaking patients or family members.

During business hours, call our main clinic number at 703-776-2986. Follow the prompts to make follow-up appointments, request refill prescriptions or to leave a message for your coordinator. Our financial coordinator can be reached at 703-776-2577.
Emergency Contact After Business Hours
Call the hospital operator at 703-776-4001 and ask to page the lung transplant coordinator on call. Please do not use this line for routine matters.
Physicians
Referring physicians may speak with members of the lung transplant team 24 hours a day, seven days a week. During business hours, contact our referral coordinator, Andrea Grajeda, at 703-776-6168 or 888-677-1778.
