Radiation Therapy Patient Forms

Your first visit will be a comprehensive consultation with one of our doctors - please arrive 30 minutes prior to complete necessary paperwork. Bring reports and CDs of any scans (CT, MRI, PET, X-rays) as well as other diagnostic reports. If you have any questions, our schedulers will assist you in determining what to bring to your visit.

Your insurance card and photo ID are needed at each visit, as well as a list of your medications (prescription and over-the-counter).

If you are a new patient at Inova Schar Cancer Institute, you may be asked to complete the forms below and bring them with you to your appointment. Your new patient coordinator will advise you prior to your appointment.
