Inova Laboratories offers flexible billing options and will bill the client, patient or third-party insurance as indicated on the test requisition form.
Client billing
An itemized statement is generated when a physician is billed for laboratory services. The invoice is alphabetized by patient name and includes a detailed listing of all procedures performed. The date of service and corresponding fees are listed.

- Date of birth
- Diagnosis
Patient (direct billing)
If direct patient billing is requested, all necessary billing information must be provided on the Inova Laboratories requisition form.

- Mailing address
- Date of birth
- Diagnosis
- Patient's signature
Patient billing will be at CPT code level.
Insurance (third-party billing)
For patients who are recipients of benefits or subscribers of third-party agencies (i.e., Medicare or insurance companies), Inova Laboratories will bill the agency directly. The patient will be billed only for services not covered by their third-party agency. Correct billing information must be provided for those patients requiring third-party billing.

- Patient's full name as listed on their insurance card
- Social Security number
- Mailing address
- Date of birth
- Diagnosis
- Patient's signature
- Photocopy of the patient's insurance card, both sides
- For Medicare, secondary payor information
- Guarantor (if patient is a minor, guarantor's demographic and billing information must be provided as well)
Third-party billing will be at CPT code level.
Please note:
If we receive a specimen without correct billing information, Inova will apply those charges to the client's bill.
If a billing instruction change is required, please notify us within 45 days of service and provide all pertinent billing information. Inova Laboratories assumes no responsibility for billing errors due to reliance on CPT codes listed in the Testing Directory. For further reference, please consult the CPT Coding Manual published by the American Medical Association. If you have any questions regarding the use of a code, please contact your local Medicare carrier.
