Your Office Visit

Patient forms

As directed by our office staff, please complete the patient forms below and bring with you to your appointment.

How do I make an appointment?

Make an appointment by calling 703-970-2670. A patient care specialist will guide you through the process, recording detailed information about your condition and demographics. Appointments with a neurosurgeon can often be made within a week, although requests to see specific neurosurgeons may take longer.

Does my insurance cover the visit?

The coverage of your visit by your insurer depends on your policy. You may be subject to a co-payment, according to your policy. Additionally, tests or imaging studies resulting from your visit are subject to insurance approval. We recommend that you contact the customer service for your insurance to learn the details of your coverage and co-pays, if any.

Inova Medical Group no longer accepts Kaiser Permanente.

If you do not have insurance, the 2010 Affordable Care Act requires all persons to carry health insurance. You can learn more about obtaining health insurance at www.hhs.gov.

What should I bring to my appointment?

  • All imaging studies and reports, such as X-rays, CT or MRI scans
  • A full list of your medications, including name, dosages, frequency and the prescribing physician. These medications will be reviewed during your clinic visit.
  • All medical records pertinent to your clinic visit from non-Inova physicians, including reports from consultations and hospital discharge summaries
  • If you have a heart condition, please bring any pertinent documentation from y our cardiologist, including history of surgery, stents, ECG or echocardiograms. This is necessary to evaluate your fitness for surgery.
  • Insurance cards and claims forms

What can I expect during my appointment?

Your neurosurgeon and dedicated staff will obtain a detailed history and perform a physical examination. Your neurosurgeon will also review your imaging studies. After obtaining all information, your neurosurgeon will meet with you to discuss your condition and make specific treatment recommendations.

If surgery is recommended, the procedure will be clearly explained, including the risks, benefits and alternatives. Our scheduling team will work with you on a surgery date. You may be asked to obtain pre-surgical labs prior to surgery and given further instructions after a date is scheduled.

However, not all conditions require surgery. Based on your condition and circumstances, your neurosurgeon will provide information on the best alternatives to surgical treatment. This may include observation, medications and/or referrals to another specialist. If follow-up appointments are needed, our scheduling staff will assist you in selecting a convenient date and time.
